Dr. Mitchell Duckworth
Dr. Mitchell Duckworth, DDS, leads the Summit Anesthesia team. Born and raised in Salt Lake City, UT, Dr. Duckworth loves the outdoors and exploring all that Utah has to offer.
He is an experienced general dentist and residency-trained dentist anesthesiologist. He graduated from the University of Southern California’s Ostrow School of Dentistry and subsequently completed a residency in anesthesia at Stony Brook University Hospital, in Long Island, New York. During his residency, he completed rotations in Cardiology, Pain Management, Ambulatory Anesthesia, Post-Anesthesia Care, as well as Internal and Pediatric Medicine.
Dr. Duckworth specializes in delivering individually tailored anesthesia to all populations, including pediatric and patients with special needs. He takes great pride in making each patient feel comfortable, while trying to understand each individual’s needs and anxieties.
He's had many opportunities to offer service through his profession in under-developed countries throughout Central and South America. These trips are the highlight of his career and he is always looking for new opportunites to help create healthy smiles.
He is certified by the Utah Division of Occupational and Professional Licensing to provide all levels of anesthesia. He is also board-eligible through the American Board of Dental Anesthesia. Dr. Duckworth is also certified in the following specialties:

Dr. Levi Taylor
Dr. Levi Taylor, DDS, is our newest Summit Anesthesia team member. Born and raised in Kaysville, Utah, he received his bachelor’s degree from Brigham Young University-Idaho. He continued his education at the University of Buffalo School of Dental Medicine in New York, where he graduated Magna Cum Laude and was invited into Omicron Kappa Upsilon (OKU), the National Dental Honor Society. He completed his residency training in dental anesthesiology at UCLA, a hospital system ranked 3rd best in the country. In addition to anesthesia training he completed training in Cardiology, Emergency Medicine, and Pediatrics.
Dr. Taylor’s favorite part about dental anesthesia is working with pediatric and special needs patients. Being able to provide a pain-free dental experience to a population that often experiences the most dental related fear and anxiety, brings a special form of gratification. As a dentist anesthesiologist he is able to adapt the anesthesia experience to each of his patients. This includes anything from a minimal sedation to general anesthesia.
Dr. Taylor has had the opportunity to serve and provide free dental care for underserved kids and adults throughout western New York and Tennessee, as well as multiple dental clinics and surgery centers throughout Los Angeles.
Of all his accomplishments, his greatest joy is being married to his wife Carolyn and being a father to their three children.
